MnCHOICES Assessor- RN/PHN or Social Worker
Unit: Home and Community Based Services Assessment
This is a replace position in the Home and Community Based Services Assessment Unit. The primary responsibility of this position will be to complete MnCHOICES Assessments for the following programs: DD, EW. AC, CADI, CAC, BI, PCA, and Rule 185.
This position will be part of an interdisciplinary team comprised of a mix of disciplines and personnel with work experience in the areas of developmental disabilities (QDDP), RN/PHN, elderly, mental health (QMHP) and physical disabilities. Selection will be given to a person with the professional credentials and/or work experience that meets a gap in the composition of the team and the education and work experience requirements in accordance with MN Statute 256B.0911, Subd. 2b (b).
We are accepting applications from certified public health nurses, registered nurses, and Social Workers with at least two years of home and community based experience and must be Merit System eligible. Preference will be given to certified public health or registered nurse applicants for this opening.
MnCHOICES Assessor must be able to complete and pass the required MnCHOICES Certified Assessor Training (MNCAT) and Certification Process.
Candidates for this position must be proficient in the use of electronic equipment and related software as MnCHOICES is electronic assessment tool only. In addition, this position requires excellent organizational skills and accuracy/timely completion of paperwork due to the high volume of assessments to be completed annually by each Assessor.
Must be Merit System eligible as a social worker, or have Public Health Certification with at least one year community based experience. Consideration may be given to an RN with two years of community based experience, based on applicant pool. Questions should be directed to Tamraa Goldenstein at X2482.
